NHS Digital are launching a new data collection programme called General Practice Data for Planning and Research.

If you wish to opt out of sharing your data, there are two ways you can do this.

1. Fill in a Type 1 opt out form and bring/send it to your surgery by 23rd June 2021. You can download the form here: Opt-out-Form. This means that your data will not be shared with NHS Digital.
2. Complete a National Data opt out form online: https://digital.nhs.uk/services/national-data-opt-out 

This ensures that whilst your data may be shared with NHS Digital, it will not be shared beyond that.

All opt outs need to be completed or with your surgery by 1st September 2021 in order to take effect before the scheme begins.
